High School Consent Forms

In addition to the general team registration requirements, each high school team must complete and submit a set of consent forms. Consent forms are required from all high school team participants including instructors, advisors, students, and others. We must receive a consent form for each team member in order to approve his/her iGEM.org user account.

In order to approve a team, we must receive the following consent forms:

  • Principal consent form for every school represented on the team
  • Participant consent form from the primary PI of the team
  • Department head consent form (if the team is working at a laboratory outside of their high school)

A team may submit their consent forms using the online consent form system OR the paper consent forms. You do not need to submit both an electronic consent form and a paper consent form. We recommend using the online consent form system if possible: you will receive immediate acknowledgment that your form was submitted, iGEM HQ will be able to process them faster, and your user account will be approved sooner.

Consent Form Deadline: July 23, 23:59 EDT

The remaining Participant Consent Forms for the student team members, advisors, instructors, and secondary PIs (if applicable) must be submitted by this deadline.

Requirement: Approved Team Name

Your team name must be approved by iGEM Headquarters BEFORE you submit your consent forms, as the team name on the consent form must match your approved team name.

Online Consent Form System

Be sure to create an iGEM.org user account first, then join your team roster. The online consent form will request your iGEM.org username, official Team Name, and Team Code. The Team Name and Team Code can be found on your Team Information page. You may access the online consent form system using the appropriate links below. After completing the form, you will receive a verification email from SmartWaiver. You will need to click on the link in the email verification in order to complete the process. Be sure to check both your inbox and spam box for this email.

All teams must complete and submit the following consent forms:

  • Principal Consent Form: Each school represented on the team must submit a principal consent form. This form is a confirmation that the school is aware of the team's participation in iGEM. Note: your principal does not need an iGEM.org user account in order to access or complete the consent form.

  • Participant Consent Form: Every team member (PI, instructor, student, advisor, etc.) must complete a participant consent form. Create your iGEM.org user account first, then click on the button to complete your consent form. The consent form will ask for your iGEM.org username and associated email address. Students: if you are under the age of 18, your parent/guardian will need to submit a signature. Submit your form as soon as possible so we can approve your iGEM.org user account, and you can gain access to the Registry as well as your team wiki!

If you are working at a university laboratory, or other laboratory outside of your high school, your team will also need to submit the following consent form:

  • Department Head Consent Form: This form is a confirmation that the department is aware of the team's participation in iGEM, and grants permission to the team to use its laboratory facilities. Note: your department head does not need an iGEM.org user account in order to access or complete the consent form.

Paper Consent Form System

We recommend using the online consent form system for faster processing, but if you are not able to access the system, you may download the paper consent forms below. Print and complete these forms, then mail them to the address at the bottom of this page.

The original hard copy of the consent forms must be sent to iGEM Headquarters at the address below. We strongly recommend shipping the consent forms via a carrier that will provide a tracking number and providing that number to documents [AT] igem [DOT] org.
