In the early stages of building the team and choosing a project, we chose to work on a project that has implications for society in a positive way. One of the aspects that is important to us is the safety and biosecurity of the project. This is why we decided to interview a pharmacist researcher in the French Armed Forces, involved in the fight against biological risks of intentional (state, bioterrorism...) or accidental origin. This soldier made us aware of the possible drifts of synthetic biology on the wrong side of the force. This exchange allowed the team to become fully aware of the role of Human Practices, and more importantly, of our responsibility as participants in a synthetic biology competition and as actors in a scientific project.

Around the world, thousands of women and persons with women's reproductive system, experience pain related to their menstruation every day. It can be slightly painful, but it should not prevent them from being able to live their daily lives. Endometriosis is a disease that affects about one in ten women. Instead of being eliminated by the vagina during menstruation, endometrial cells migrate to other organs. Endometriosis has a large number of symptoms, making it more of a syndrome than a disease. We don't know exactly how it appears or what causes it. But one thing is certain: it causes pain and is taboo. The psychological burden is also heavy to bear for the patients. But above all, the diagnosis of endometriosis is long and sometimes requires the use of expensive or sometimes inaccurate invasive examinations. This is what motivated EndoSeek to try to find a solution to these different problems.

But in order to complete a research project, staying in the lab is not sufficient. Actually there are many different aspects to study. How could the project have an impact on society ? How do patients with Endometriosis see our project ? Moreover, a project can have a really different impact on society based on the applications of it. That’s why we tried to adapt it by exchanging with stakeholders : doctors , writers and others since it appeared to us there was no global consensus about it : indeed we developed our project in parallel with our interviews and the testimonies we received.

We focused our approach on giving a voice to those concerned because it seemed essential to us that these people, mostly women but also people in transition, reappropriate the word and the discourse around this disease. This first step in the development of our Human Practices allowed us to become aware of the issues surrounding this disease through the prism of those who are first involved in it. Indeed, while collecting the different testimonies, we came up against a problem affecting women that is broader than endometriosis, namely the minimization and trivialization of pain/symptoms. Some women do not feel legitimate to consult a health professional. In addition, sexuality and women's health are subjects that are not often discussed within the family or at school, which contributes to a certain lack of knowledge and difficulty in talking about them. It also depends on the culture. However, the cultural stigma around endometriosis and fertility preservation is still present in many countries. The situation in rural areas in India for example is quite complicated as menstruation is a real taboo issue. So how can women know about endometriosis if they can’t even talk about menstruations?

Various testimonies were received and it appeared that the time for the diagnosis to be established was very long, on average 7 years since the beginning of the symptoms. According to these testimonies, this is due to different factors. In fact, they found that the specialists directed the diagnosis more towards other health problems.

Among these testimonies one idea comes up:

"if I hadn't taken matters into my own hands I could have waited a lot longer".

This sentence tends to show that there is a lack of expertise and, more generally, that the health care system is deficient in the way that there is medical wandering during the course of care."Some general practitioners do not know the disease", one woman told us. This is confirmed by the fact that graduate medical students have only been taught about endometriosis since 2020.
There is both a medical errancy linked to a lack of expertise or this diagnosis is "left aside" and also to erroneous diagnoses, as in the case of one woman who said that she had been referred to several wrong diagnoses even though she was being followed by several specialists (gynaecologist, radiologist).
In addition, in view of the complexity of establishing a clear and reliable diagnosis, several testimonies share their enthusiasm for a potential molecular diagnosis that could be developed.

These initial testimonials were a starting point. However, they did not represent a sufficiently varied study group (women affected by this disease). It was therefore necessary to have the opinion of a wider and more representative public, not only consisting of women who already knew about endometriosis, given that the disease is part of a wider context.
A survey was then created to find out more about the knowledge of endometriosis within the iGEM community. The iGEM community allows us to reach an audience that is not only scientifically literate, but also international, mixed, and with diverse backgrounds and experiences.
A total of 135 responses were collected. The respondents came from 32 different countries (France, Germany, India, Greece, Austria, Sweden, United States, Mexico, Taiwan, Bolivia, Spain, Bulgaria, Estonia, Russia, Canada, Indonesia, Belarus, Netherlands, South Korea, Brazil, United Kingdom, Hong Kong, Australia, Finland, Peru, Switzerland, Turkey, Denmark, China, Lithuania, Puerto Rico and Azerbaijan).

It was found that although few iGEMers (23.1%) do not know about the disease, a larger proportion (36.6%) have heard of it but do not know what it is. In order to verify that people are making their thoughts explicit, we asked for a short explanation of the disease. In general, people answered that it was a disease related to the uterus but did not give any further details. This may suggest that respondents are not sure what it is. Other responses were lists of some symptoms but without giving the definition of endometriosis. In addition, many wrong answers were given: confusion with bleeding outside the menstrual period, inflammation, cysts, endometrial damage, endometrial infection, a condition that makes the endometrium thicker than normal, a malformation of the uterus, autoimmune disease that makes the uterus bleed abnormally...
Thus, these varied answers prove that endometriosis is not yet sufficiently known by the general public. Fortunately, there are still some correct answers.

Not only did the team work on the impact of the project on society by interviewing patients, associations, and doctors, but we also studied the History of Endometriosis in Art. A whole research work was made in order to see how Endometriosis was represented and therefore how it was seen by the artists and by the society.
This year, we are lucky to have a team member from the Ecole du Louvre who could share with us and make this study.
In order to learn about endometriosis not only on a scientific scale but also on a human scale (as it is the project of Human Practices), we did some research about the way endometriosis was presented in History and history of Arts. We did as well research about endometriosis in History from Ancient Egypt until the 20th century that we shared with other IGEM teams during the Zoom event on the 11 of September 2021, The French Meet-up. During this presentation we focused on special parts of endometriosis in History, for example with the eber papyrus, an ancient egyptian text, dealing for the first time in History about endometriosis-related symptoms or a painting from Pierre Aristide André Brouillet depicting a clinical lesson in the parisian hospital named La Pitié Salpétrière where a class about hysteria was taught.

These initial discussions allowed us to see that endometriosis was not well known, for several reasons. We therefore had to talk directly to professionals in the medical field who were already familiar with endometriosis.

One of the people we spoke to in this discussion was Professor François Golfier. He is the President of the EndAURA Network (Endometriosis Auvergne-Rhône-Alpes) and the Head of the Department of Gynecological and Oncological Surgery - Obstetrics at the University Claude Bernard Lyon 1 (University Hospital). One of the reasons why we contacted him is that he is also a member of the scientific committee of EndoFrance.
Indeed, like the other doctors on this committee, he believes that endometriosis is a public health issue that is poorly managed and in particular the surgical indication needs to be improved, as does the quality of the surgical procedures.

We asked him how long it takes from the first symptoms to the final diagnosis. According to him, the general view is that it takes about 7 years. He also told us about the advances made by artificial intelligence (AI), in particular ZIWIG (an open access platform on the Internet). Indeed, one of the objectives of this platform is to drastically reduce the diagnostic delay through the use of AI, based on a questionnaire drafted by some twenty French experts in endometriosis and filled out by women concerned by pelvic pain and who wonder whether they have endometriosis (more than 12,000 patients since January 2021, the launch date). The use of AI will give the possibility to give a probability of endometriosis that will evolve over time thanks to the "machine learning" used. This is a major advance that will most certainly be brought about by the scientific work in progress on this subject.

Prof. François Golfier recommends a diagnosis including both an interrogation about the symptoms, a gynecological examination and/or MRI. Indeed, the diagnosis may sometimes require recourse to surgery by laparoscopy when the imaging is inconclusive, but an expert radiologist with MRI can diagnose most endometriosis without the need for laparoscopy. The use of ZIWIG AI is a new weapon under development. Effective biological diagnosis would be the holy grail for the research professionals involved.

We also watched a video of Erick Petit on the YouTube platform and asked François Golfier for a reaction. In this video, Dr. Petit indicated that the pain mentioned by adolescents during their first year of menstruation is a reliable diagnosis of endometriosis. The doctor replied that he does not agree with the idea. He said that this advice would expose a significant risk of false positives. These young women would be told that "they have endometriosis and this would dramatically affect their quality of life. The doctor also mentioned that imaging tests, especially MRIs, should be done by experts. Indeed, we sometimes have false positives due to a lack of expertise of some radiologists. In the context of our project, it is therefore probably wiser not to propose this diagnostic test during the first year of menstruation unless the patient complains of very severe pain.

According to him, a less invasive diagnosis than laparoscopy is the inevitable future in the short or medium term, ideally a blood sample (plasma or serum). Finally, Prof. François Golfier has heard about specific biological markers for endometriosis and finds our idea of miRNAs interesting. This video and the words of the different interviewees therefore confirm our idea of diagnosis by blood sampling but also that many errors can be made when reading the imaging, something we have confirmed by the doctors and the testimonies.

We interviewed Professor Pierre COLLINET, (Gynaecologist-obstetrician for 15 years) from CHU Jeanne de Flandres in Lille. We talked about how we had to be careful and choose between a diagnosis and a screening test since the direct design experiment of our project depended on it . We thought about it and it gave us the idea to talk about this problem during our work about ethics for our Partnership with Lund (iGETHICS).
Professor Pierre Collinet says he has seen an increase in requests for care for endometriosis-related problems over the last ten years. It is because of this increase in requests that he decided to start his own practice, as his colleagues do not necessarily want to work in this type of complex surgery. With other practitioners, he created a RCP in Lille, where there are now many different approaches: organization of care, communication about the disease, training related to the latest news about the disease. Moreover, more than 10 years ago, he met the EndoFrance association with whom he agreed to work, notably for the ethical aspect that is important to him, and also for the scientific approach, which he believes is rare for a patient association. He is therefore a member of the scientific committee of EndoFrance. Being close to the patients of this association allows him to understand them better and to be more human towards them.

Following this interview, we made several observations. It raised issues that we had not thought about, particularly the distinctions between screening tests and diagnostic tests. Indeed, a diagnostic test is carried out on a case-by-case basis, often following complaints from the patient. The screening test, on the other hand, is carried out on a larger scale and systematically. Professor Collinet told us that a screening test that proves to be positive, but which was carried out without the patient's verbal testimony, could cause the patient to experience stress, even if she did not think she had endometriosis. Furthermore, on too large a scale, screening could lead to a high rate of false positives, which would then have consequences for the patients. Screening would not indicate the stage of advancement of the disease and could cause a certain amount of panic in these women, on the one hand, with regard to the seriousness of the possible symptoms and, on the other hand, with regard to their desire to have a child.

In addition, the gynecological surgeon Michel Canis discussed this issue, pointing out that laparoscopy was "outdated" and that it was based on imaging techniques (MRI / vaginal endoscopy). We were concerned about this because at the beginning we looked at the scientific literature which indicates that laparoscopy is the golden standard [...] in terms of diagnosis of this disease. Similarly, the testimonies show that laparoscopy has not been recommended as a first-line diagnosis. These remarks illustrate the interest of human practices and, in particular, the need to build a bridge between the lab and human practices.

We interviewed by telephone Professor Michel Canis, a gynecological surgeon with a close interest in endometriosis. He is a member of the Estaing University Hospital and in 1982 set up the first congress on the disease, or rather on the "syndrome", as he prefers to call endometriosis. Indeed, a syndrome is a set of symptoms whose cause is unknown.
He relies in particular on endoscopic surgeries, which he describes as the "trademark" of the dedicated department in Clermont-Ferrand. After a reminder of the biological definition of endometriosis, we discussed the difficulty of detecting a lesion related to this syndrome, or of locating endometrial tissues that are less distant from the site of origin. He first explained that laparoscopy is an "outdated" method of diagnosis and that advances in imaging, particularly MRI and CT scans, allow a diagnosis to be made in the majority of cases.
He indicated that laparoscopy is indeed considered as a "golden standard" in the sense that tissues are taken and analyzed in anatomopathology, which makes it possible to certify the presence of endometrial tissue at a distance from the endometrium. When he started talking about endometriosis markers, he explained that with the advent of molecular and genomic biology in the 2000s and the availability of a "tissue tech" (a database of endometrial tissues), we expected to see major progress in this syndrome. However, 21 years later, there are still no clear markers to detect this pathology, which shows how difficult it is to find a marker.
Moreover, the marker must meet several strict criteria, on the one hand it must not be found in other pathologies, and on the other hand it must not be disturbed by a contraceptive method.
A molecular diagnosis could be useful as there are different types of endometriosis. Some endometrial tissues are easy to categorize as originating from the endometrium (those in the kidneys for example) but some tissues in the uterine muscle are difficult to identify as related to endometriosis (even with histological research), hence the usefulness of a future diagnosis based on the detection of markers In addition, a marker would be useful to explain the "why" of the pathology. The Professor drew attention to the fact that one "manufactures failure with information", and that one must be careful to establish an image aid but not necessarily a systematic screening.
He shares interesting leads such as Hughes Taylor's "endometrial stem cells" for detection, and mentions the miRNA lead in "Cell" of May 2021 that has not been proven to date.

This interview made us aware of the magnitude of this problem and the multiple obstacles that research can face. The choice of a detection method with markers is complex because these markers must only be present in the case of endometriosis.

Fanny Robin, whom we interviewed via Zoom on 05/07/2021, wrote the comic books "Juste une endométriose" (2021) as well as "En coloc avec l'endométriose" (2020) with May Fait Des Gribouillis. Suffering from this disease, she expresses situations from the daily life of patients, in order to reveal the daily life of women with endometriosis, always with a touch of humor. Fanny Robin also manages an Instagram account (@chere.endometriose) in which she shares her daily life with the disease, but with positivity according to her.

It seemed relevant to have a testimonial from a patient by evoking the feedback from the interviews we had done with the doctors. Moreover, our EndoSeek project aims to help these patients and therefore to touch them. Our exchanges with this representative of patients suffering from this disease allowed us to better understand, humanly speaking, the problems of the patients.

At the beginning of her testimony, she indicated that too often endometriosis is approached from the angle of infertility, as if, in the collective unconscious, women were seen as having to be productive, particularly with regard to being able to give birth. However, as she rightly points out, when we talk about endometriosis, it is not only a question of fertility, but of other problems, namely physical suffering in the first place, but also the psychological impact, both for the individual and for any spouses and/or family members. These remarks echo our introductory sentence on the relevance of giving back the witness of speech to the persons concerned.

According to her, a molecular diagnosis would be useful as long as it is detectable even when taking the pill; indeed, taking the pill masks the symptoms, so we need a reliable marker that is insensitive to the effect of the pill. We showed her a draft of our logo, and this exchange allowed us to improve it so that people with the disease could identify and feel represented by it.

In this spirit, but particularly related to art, we also looked for 21st century women artwork dealing with endometriosis and one especially came to our mind, "Break my pelvic sorcery" from the Australian artist Eugenie Lee. She made a pelvic dispositive device in the shape of a belt that causes sensations mimicking pelvic pain, the main and one of the many symptoms of endometriosis. The title was evocative here with a link between the pain caused and witchcraft, an inexplicable phenomenon, mostly negative connotations, but also little known and feared, another comparison with the way in which the disease is swept from the diagnosis by some health professionals.This device used by the artist was originally used for therapeutic purposes, but it was reconfigured with the aim of causing pain.

To summarize, all the interviews, testimonies and research conducted by the Human Practices department have allowed us to develop our EndoSeek project in such a way as to bring it closer to the people concerned and by anchoring it in a psycho-social context. We discussed the project and listened to their advice, remarks and criticisms.Our main concern during this year is that the patients can identify with the work we have produced in this project. The exchanges with Mrs. Robin allowed us, for example, to co-construct a project logo incorporating elements symbolizing this disease such as the yellow ribbon, symbol of endometriosis. Discussions with health professionals helped our project to evolve from a diagnostic to a screening perspective, as we did not initially take into account the psychological care of patients when they discovered this disease, or potentially induce a panic movement among women by proposing a public test.

Our initial idea was that of a test to detect endometriosis using synthetic biology methods. Then we detailed the project with the choice of a serological test, then the conditions under which it would be carried out, namely following an exchange and the recommendation of a doctor. We then expanded it by choosing to offer this test not as a screening test but as a diagnostic test. We thought it was less invasive than some of the current tests used in the diagnosis of endometriosis. However, serological sampling, even if it is less invasive than laparoscopy, is still an invasive procedure. But the latter offers a much clearer reading than imaging.

This adventure would not have been possible without the commitment of the members of the team, but also and above all of the various doctors who shared their thoughts with us and of the patients who had to confide in us, a difficult thing at times, about their relationship with the disease. In order to respond to current issues and problems, it is necessary to evaluate a significant number of parameters. The discipline of Human Practices and even more so of Integrated Human Practices was therefore fundamental.

Au début de la constitution de l’équipe et du choix d’un projet, nous avons choisi de vouloir travailler sur un projet dont les tenants et les aboutissants ont des implications sur la société de manière positive. L’un des aspects qui nous tient à cœur est la sécurité et la biosécurité du projet. C’est pourquoi, nous avons décidé d’interroger un Pharmacien chercheur dans les Forces Armées françaises impliqué dans la lutte contre les risques biologiques d’origine intentionnelle (étatique, bioterrorisme…) ou accidentelle. Ce militaire nous a sensibilisé aux dérives possibles de la biologie synthétique du mauvais côté de la force. Cet échange a permis à l’équipe de pleinement prendre conscience du rôle des Human Practices, et plus grandement, de notre responsabilité en tant que participant à un concours de biologie synthétique et en tant qu’acteur d’un projet scientifique.

Dans le monde, des milliers de femmes expérimentent chaque jour des douleurs liées à leur menstruations. Elles peuvent être légèrement douloureuses mais ne doivent pas empêcher de pouvoir vivre son quotidien. L’endométriose est une maladie qui affecte environ une femme sur dix. Au lieu d’être éliminées par le vagin lors des menstruations, des cellules de l’endomètre migrent vers d’autres organes. L’endométriose comprend un nombre important de symptômes, ce qui en fait plus un syndrôme qu’une maladie. On ne sait pas exactement comment elle apparaît ou quelles sont les causes de son déclenchement. Mais une chose est plus sûre, elle provoque douleurs et tabous. Le poids psychologique est aussi lourd à porter pour les patientes. Mais surtout, le diagnostic de l’endométriose est long et nécessite parfois le recours à des examens invasifs, coûteux ou parfois imprécis. C’est ce qui a motivé EndoSeek à tenter de chercher une solution face à ces différents problèmes.

Mais dans un projet de recherche, la partie laboratoire n’est pas suffisante, d’autres aspects sont à étudier. Comment le projet a-t-il un impact sur la société ? De quelle manière les patientes atteintes d’endométriose voient notre projet ? De plus, un projet peut avoir différents impacts sur la société selon ses applications. C’est pourquoi il est nécessaire d’adapter le projet selon les échanges avec les différents interlocuteurs.

Nous avons axé notre démarche en donnant la parole aux concernées car il nous semblait essentiel que ces personnes, en majorité des femmes mais aussi des personnes en transition, se réapproprient la parole et le discours autour de cette maladie. Ce premier jalon dans le cheminement de nos Human Practices nous permettait de prendre conscience des enjeux gravitant autour de cette maladie sous le prisme des primo-impliquées dans cette maladie. En effet, en recueillant les différents témoignages, nous nous sommes heurtés à une problématique touchant les femmes, plus large que l’endométriose à savoir la minimisation et la banalisation des douleurs/symptômes. Certaines femmes ne se sentent pas légitime de consulter un professionnel de santé. De plus, la sexualité et la santé féminine sont des sujets peu abordés dans la cellule familiale ou scolaire, ce qui contribue à une certaine méconnaissance et à une difficulté à en parler.

Différents témoignages nous sont parvenus et il est apparu que le temps pour que le diagnostic soit établi était très long, en moyenne 7 ans depuis le début des symptômes. D’après ces témoignages, cela est dû à différents facteurs. En effet, elles ont constaté que les spécialistes orientaient plus le diagnostic vers d’autres problèmes de santé.

Parmi ces témoignages une idée revient :

“ Si je n’avais pas pris les choses en main j’aurais pu attendre encore longtemps”.

Cette phrase tend à montrer qu’il existe un manque d’expertise et plus globalement que le système de santé est lacunaire dans la mesure où il existe une errance médicale lors du parcours de soins. “Certains médecins généralistes ne connaissent pas la maladie” nous révèle une femme.Cela se vérifie dans les faits, on constate en effet que les étudiants de médecine de 2e cycle bénéficient des enseignements sur l’endométriose seulement depuis 2020.
Il y a à la fois une errance médicale d’une part liée à un manque d’expertise où ce diagnostic est “laissé de coté”, et d’autre part à des diagnostics erronés, à l’image d’une femme qui fait part du fait qu’elle a été aiguillée vers plusieurs mauvais diagnostics alors qu’elle était suivie par plusieurs spécialistes (gynécologue, radiologue). En outre, en regard de la complexité d'établir un diagnostic clair et fiable, plusieurs témoignages nous font part de leur enthousiasme vis-à-vis d’un potentiel diagnostic moléculaire qui pourrait être développé.

Ces premiers témoignages furent un point de départ. Or ces derniers ne représentaient pas un groupe d’étude assez varié (en l'occurrence, un public féminin atteint par cette maladie). Il était alors nécessaire d’avoir l’avis d’un public plus large et représentatif, n’étant pas uniquement constitué de femmes connaissant déjà l’endométriose, étant donné que la maladie s’inscrit dans un contexte plus large.

Un sondage a alors été créé afin d’en savoir plus sur la connaissance de l’endométriose au sein de la communauté iGEM. Cette dernière permet de toucher un public, certes initié aux sciences, mais surtout international, mixte, avec des expériences et des horizons divers.
Un total de 135 réponses a été collecté. Il est apparu que, même si peu d’iGEMers (23.1 %) ne connaissent pas la maladie, une part plus importante (36,6%) en a déjà entendu parler mais ne sait pas de quoi il s’agit. Afin de vérifier que les personnes explicitent leurs pensées, nous avons donc demandé une courte explication sur cette maladie. En général, les personnes répondent qu’il s’agit d’une maladie liée à l’utérus mais sans donner plus de détails. Cela peut laisser entendre que les répondants ne savent pas exactement de quoi il s’agit. D’autres réponses sont des listes de certains symptômes mais sans donner la définition de l’endométriose. De plus, beaucoup de fausses réponses ont été données: une confusion avec des saignements hors de la période de règles, une inflammation, des kystes, des dommages de l’endomètre, une infection de l’endomètre, une condition faisant que l’endomètre est plus épais que la normale, une malformation de l’utérus, maladie auto immune qui fait que l’utérus saigne anormalement… Ainsi, ces réponses variées prouvent que l’endométriose n’est pas encore assez connue du grand public. Heureusement, il reste tout de même une partie de réponses justes.

Nous avons non seulement travaillé sur l’impact présent du projet mais il était aussi pertinent d’avoir un regard sur le passé. Un travail de recherche a donc été fait sur la manière dont l’endométriose était abordée dans l’art et l’histoire, et sur la manière dont elle était perçue par les artistes et la société. A la suite de ce travail de recherche, un article à été rédigé présentant l’histoire de l’endométriose de l’ère Néolithique et ce jusqu’au 21ème siècle, en abordant des points historiques mais aussi des oeuvres qui montrent la perception de l’endométriose au fil du temps. Une présentation a également été faite, lors d’un événement virtuel sur la plateforme Zoom, le 11 septembre 2021, dans le cadre du French meet-up.

Ces premiers échanges nous ont permis de voir que l’endométriose était assez méconnue, et ce pour plusieurs raisons. Il fallait donc aller discuter directement avec des professionnels du milieu médical qui connaissaient déjà l’endométriose.

Un de nos interlocuteurs dans cette discussion fut le Professeur François Golfier. Il est le Président du Réseau EndAURA (Endométriose Auvergne-Rhône-Alpes) et le Chef du Service de Chirurgie Gynécologique et Oncologique - Obstétrique à L’Université Claude Bernard Lyon 1 (Hôpitaux Universitaires). Une des raisons pour laquelle nous l’avons contacté est qu' il est également membre du comité scientifique d’EndoFrance.
En effet, comme les autres médecins de ce comité, il estime que l’endométriose est une thématique de santé publique mal prise en charge et notamment l’indication chirurgicale est à améliorer, tout comme la qualité des gestes opératoires.

Nous lui avons demandé, selon lui, combien de temps se passait entre les premiers symptômes et le diagnostic final. D’après lui, le discours général est qu’il faut environ 7 ans. Il nous a également parlé des avancées réalisées notamment par l’intelligence artificielle (IA), en particulier ZIWIG (plateforme en accès libre sur Internet). En effet, cette plateforme a parmi ses objectifs de réduire le délai diagnostique de façon drastique par l’utilisation de l’IA, à partir d’un questionnaire, rédigé par une vingtaine d’experts français en endométriose et rempli par les femmes concernées par des douleurs pelviennes et qui se posent la question de l’existence d’une endométriose chez elle (plus de 12 000 patientes depuis janvier 2021, date de lancement). L’utilisation de l’IA va donner la possibilité de donner une probabilité d’endométriose qui évoluera au fil du temps grâce au “machine learning” utilisé. Il s’agit d’un progrès majeur que viendront très certainement apporter les travaux scientifiques en cours sur ce sujet.
Pr. François Golfier préconise un diagnostic comportant à la fois un interrogatoire au sujet des symptômes, un examen gynécologique orienté et/ou IRM. En effet, le diagnostic peut parfois nécessiter de recourir à la chirurgie par coelioscopie lorsque l’imagerie n’est pas concluante mais un expert radiologue en IRM fait le diagnostic de la plupart des endométrioses, sans nécessité de recours à la coelioscopie. L’utilisation de l’IA de ZIWIG est une arme nouvelle en cours de développement. Un diagnostic biologique efficace serait le graal des professionnels chercheurs concernés.

Nous avons également visionné sur la plateforme YouTube, une vidéo d’Erick Petit, puis nous avons demandé une réaction à François Golfier . Dans cette vidéo, le docteur Petit indiquait que les douleurs mentionnées par les adolescentes lors de leur première année de règles sont un diagnostic fiable de l’endométriose. Le docteur nous a répondu qu’il n'adhère pas à l’idée. Il précise que cet avis exposerait à un risque important de faux positif. On affirmerait à ces jeunes femmes qu’elles “sont atteintes d’endométriose ce qui affecterait la qualité de vie de façon dramatique”. Le docteur a également mentionné que les examens d’imagerie et principalement l’IRM devaient être réalisés par des experts. En effet, on a parfois des faux positifs dus à un manque d’expertise de certains radiologues. Dans le cadre de notre projet, il est donc probablement plus judicieux de ne pas proposer ce test de diagnostic dès la première année de règle sauf si la patiente se plaint de très fortes douleurs.

D’après lui, un diagnostic moins invasif que la laparoscopie est l’avenir incontournable à court ou moyen terme, dans l’idéal un prélèvement sanguin (plasma ou sérum). Enfin, Pr. François Golfier a entendu parler des marqueurs biologiques spécifiques de l’endométriose et trouve que notre idée des miRNA est intéressante.Cette vidéo et les mots des différents interlocuteurs confirment dès lors notre idée de diagnostic par prélèvement sanguin mais également celui que beaucoup d’erreurs peuvent être commises sur la lecture de l’imagerie, chose que nous avons confirmée par les médecins et les témoignages.

Nous avons également interviewé le professeur Pierre Collinet, (Gynécologue-obstétricien depuis 15 ans au CHU Jeanne de Flandres à Lille). Nous lui avons dit que nous devions être prudents et choisir entre un diagnostic et un test de dépistage puisque le design complet de notre projet en dépendait. Cela nous a alors donné une idée, celle de parler de ce problème sur un travail concernant l'éthique, et intitulé iGETHICS, pour notre partenariat avec l’équipe iGEM de l’Université de Lund. Le Professeur Pierre Collinet dit voir augmenter depuis une dizaine d’années les demandes de soins pour des problématiques liées à l’liée à endométriose. C’est à cause de cette hausse des sollicitations qu’il a décidé de se lancer, car ses confrères ne veulent pas nécessairement travailler dans ce genre de chirurgie complexe. Avec d’autres praticiens, il a créé une RCP à Lille, où il y a désormais beaucoup de démarches variées : organisation de soins, communication sur la maladie, formation liés à l’actualité de la maladie. De plus, il y a plus de 10 ans, il a rencontré l’association EndoFrance avec qui il a accepté de travailler, notamment pour le côté éthique qui lui tient à cœur, et également pour ce qui est de la démarche scientifique, qui d’après lui est rare pour une association de patientes. Il est donc membre du comité scientifique d’EndoFrance. Avoir une proximité avec les patientes de cette association lui permet de mieux les comprendre et d’être plus humain envers elle.

A la suite de cette interview, nous avons fait plusieurs constats. Cela a soulevé des problématiques auxquelles nous n’avions pas pensé, notamment sur les distinctions entre test de dépistage et test de diagnostic. En effet un test de diagnostic s'effectue au cas par cas bien souvent après des plaintes émises par la patiente. Le test de dépistage, au contraire, s’effectue lui à une plus grande échelle et systématiquement. Le professeur Collinet nous a indiqué qu’un test de dépistage qui s’avèrerait positif mais qui aurait été réalisé sans un témoignage verbal de la patiente, pourrait lui faire expérimenter un état de stress, quand bien même cette dernière ne pensait même pas être atteinte d’endométriose. De plus, à une trop grande échelle, le dépistage pourrait amener à un taux important de faux positifs , qui auraient alors des conséquences sur les patientes. Le dépistage en effet n’indiquerait pas le stade d’avancement de la maladie et pourrait être à l’origine d’une certaine panique chez ces femmes d’une part par rapport à la gravité des symptômes éventuels et d’autre part quant à leur souhait notamment d’avoir un enfant.

En complément le chirurgien gynécologue Michel Canis discutait en ce sens en pointant le fait que la laparoscopie était “dépassée” et qu’on se basait sur des techniques d’imageries (IRM / endoscopie vaginale ). Cela nous a interpellés car au départ nous nous sommes penchés sur les publications scientifiques qui indiquent au contraire que la laparoscopie est le golden standard [..] en termes de diagnostic de cette maladie. De même les témoignages montrent que la laparoscopie n’a pas été préconisée en première intention de diagnostic. Ces remarques illustrent bien l'intérêt des Human Practices et, en particulier, la nécessité de réaliser un pont entre le laboratoire et les Human Practices.

Nous avons donc pu également interviewer le Professeur Michel Canis, chirurgien gynécologue s’intéressant de près à l'endométriose. Il fait partie du CHU Estaing et a mis en place en 1982 le premier congrès sur la maladie, ou plutôt sur le « syndrome », comme il préfère qualifier l'endométriose. En effet, un syndrome est un ensemble de symptômes dont on ne connaît pas la cause.
Il se base notamment sur des chirurgies endoscopiques qu’il qualifie de « marque de fabrique » du service dédié à Clermont-Ferrand. Après un rappel sur la définition biologique de l’endométriose nous avons discuté de la difficulté à déceler une lésion liée à ce syndrome, ou à repérer des tissus de l’endomètre qui sont plus moins distants du lieu d’origine. Il explique tout d’abord que la laparoscopie est une méthode de diagnostic “dépassée” et que les progrès de l’imagerie notamment IRM et Scanner permettent à eux seuls de poser un diagnostic dans la majorité des cas. Cette remarque, bien que répétitive, montre bien la nécessité de toujours trouver de nouvelles méthodes.
Il indique qu’effectivement on considère la laparoscopie comme « golden standard » dans le sens où des tissus sont prélevés et analysés en anatomopathologie ce qui permet de certifier la présence de tissu endométrial à distance de l’endomètre. En commençant à parler de marqueurs de l’endométriose, il explique qu’avec l’avènement de la biologie moléculaire et génomique dans les années 2000 puis la mise à disposition d’un « tissue tech » (une banque de données de tissus endométriaux) on s’attendait à d’importantes progressions concernant ce syndrome. Or, 21 ans plus tard, il n’y a toujours pas de marqueurs qui se dégagent clairement pour détecter cette pathologie, ce qui montre à quel point trouver un marqueur n’est pas évident. De plus, le marqueur doit répondre à plusieurs critères stricts, d’une part il ne doit pas être retrouvé dans d’autres pathologies, et ne doit pas être perturbé par une méthode de contraception.
Un diagnostic moléculaire pourrait être utile dans la mesure où il existe différents types d’endométriose. Certains tissus endométriaux sont faciles à catégoriser comme provenant de l’endomètre (ceux au niveau des reins par exemple) mais certains tissus au niveau du muscle utérin sont difficiles à identifier comme liés à l’endométriose (même avec une recherche histologique), d’où l’utilité d’un futur diagnostic basé sur la détection de marqueurs. De plus, un marqueur serait utile pour expliquer le « pourquoi » de la pathologie.
Le Professeur attire l’attention sur le fait que l’on « fabrique l’échec avec de l’information », et qu’il faut veiller à établir une aide aux images mais pas nécessairement un dépistage systématique. Il nous fait part de pistes intéressantes telles que les « cellules souches endométriales » de Hughes Taylor pour la détection, et évoque la piste des miRNA dans « Cell » de mai 2021 qui n’a pas apporté ses preuves jusque-là.

Cette interview nous a permis de prendre conscience de l’ampleur de ce problème et des multiples obstacles auxquels la recherche peut se heurter. Le choix d’une méthode de détection avec des marqueurs est complexe car ces derniers ne doivent être présents que dans le cas de l’endométriose.

Fanny Robin, que nous avons interviewée via Zoom le 05/07/2021, a écrit les bandes dessinées “Juste une endométriose” (2021) ainsi que “En coloc avec l’endométriose” (2020) avec May Fait Des Gribouillis. Atteinte de cette maladie, elle exprime des situations du quotidien de patientes, afin de révéler le quotidien des femmes atteintes d’endométriose, toujours avec une touche d’humour. Fanny Robin gère également un compte Instagram (@chere.endometriose) dans lequel elle partage son quotidien avec la maladie, mais avec positivité selon ses dires.

Il semblait pertinent d’avoir un témoignage d’une patiente en évoquant les retours des interviews que nous avions réalisées avec les médecins. De plus, notre projet EndoSeek a pour but d’aider ces patientes et donc de les toucher. Nos échanges avec cette représentante de patientes atteintes de cette maladie nous a permis de mieux comprendre humainement parlant. Plus encore, il semblait pertinent d’avoir un témoignage d’une patiente en évoquant les retours des interviews que nous avions réalisés avec les médecins.
En débutant son témoignage elle indique que bien trop souvent l’endométriose est abordée sous l’angle de l’infertilité, comme si, dans l’inconscient collectif la femme était perçue comme devant etre productive et notamment en ce qui concerne le fait de pouvoir enfanter. Or comme elle le souligne justement, quand on parle d'endométriose, il n'est pas uniquement question de fertilité, mais d’autres problématiques à savoir la souffrance physique en premier lieu mais également l’impact psychologique à la fois pour la personne mais aussi pour les conjoints éventuels et/ou l’entourage . Ces remarques font écho à notre phrase d’introduction sur la pertinence de redonner le témoin de parole aux personnes concernées.

D’après elle, un diagnostic moléculaire serait utile pourvu que cela soit détectable même avec une prise de pilule; en effet prendre la pilule masque les symptômes et il faudrait donc un marqueur fiable qui soit insensible à l’effet de la pilule. Nous lui avons montré une ébauche de notre logo, et cet échange a nous a permis de l’améliorer de manière à ce que les personnes atteintes puissent s’identifier et se sentent représentées à travers celui-ci.

Dans cet esprit, nous avons aussi cherché des témoignages artistiques de femmes atteintes par l’endométriose. Un des témoignages est celui de l’artiste australienne Eugenie Lee dans son oeuvre intitulée ‘’ Break my pelvic sorcery’’, qui évoque un lien important entre la douleur causée par la sorcellerie, chose inexplicable peu connue et craint, généralement connoté de manière négative, tout comme l’endométriose. L’artiste a réalisé un dispositif constitué d'une ceinture abdominale qu’on place sur le pelvis et qui mime de fortes douleurs. Cette œuvre est expliquée plus en détails dans l’article réalisé au cours du projet sur l’endométriose dans l’histoire et l’histoire de l’art. Pour conclure, l’ensemble des interviews, des témoignages et des recherches menés par le pôle Human Practices nous ont permis de faire évoluer notre projet EndoSeek de manière à ce qu’il se rapproche au mieux des personnes concernées et en ancrant celui-ci dans un contexte psycho-social. Nous avons discuté sur le projet et écouté leur conseil, leurs remarques et les critiques.Notre préoccupation première pendant cette année est que les patientes puissent s’identifier à travers l’ensemble des travaux que nous avons produits dans ce projet. Les échanges avec Mme Robin nous ont permis par exemple de co-construire un logo de projet incorporant des élèments symbolisant cette maladie comme le ruban jaune, symbole de l’endométriose. Discuter avec des professionnels de santé a fait évoluer notre projet du prisme du diagnostic à celui du dépistage car nous n’avons pas pris en compte au départ la prise en charge psychologique des patientes lors de la découverte de cette maladie, ou encore induire potentiellement un mouvement de panique parmi les femmes en leur proposant un test grand public.
Notre idée de départ était celle d’un test de détection de l’endométriose en utilisant des méthodes de biologie synthétique. Puis nous avons détaillé le projet avec le choix d’un test sérologique, puis les conditions dans lequel celui-ci serait réalisé, à savoir à la suite d’un échange et à la recommandation d’un médecin. Nous l’avons ensuite étoffé en faisant le choix de proposer ce test non en tant que test de dépistage mais en tant que test de diagnostic. Nous le pensions moins invasif que certains examens actuels utilisés dans le diagnostic de l’endométriose. Toutefois le prélèvement sérologique, même s’il est moins invasif que la laparoscopie reste tout de même un acte invasif. Mais ce dernier offre une possibilité de lecture beaucoup plus claire que l’imagerie.

Cette aventure n’aurait pu être possible sans l’engagement des membres de l’équipe mais aussi et surtout des différents médecins qui nous ont partagé leur réflexion et de patientes qui ont dû se confier, chose difficile parfois, sur leur rapport à la maladie. Pour répondre à des enjeux et des problématiques actuels, il est nécessaire d’évaluer un nombre conséquent de paramètres. La discipline des Human Practices et encore plus des Integrated Human Practices était donc fondamentale.

В самом начале выбора проекта мы решили работать над проектом, все плюсы и минусы которого имеют положительные последствия для общества. Один из важных для нас аспектов - это безопасность и биозащищенность проекта. Вот почему мы решили взять интервью у фармацевта-исследователя из Вооруженных сил Франции, занимающегося борьбой с преднамеренными биологическими рисками (государственный, биотерроризм и т. Д.) или рисками не несущими такой характер. Этот обмен позволил команде полностью осознать роль человеческих практик и, в более широком смысле, нашу ответственность как участника конкурса по синтетической биологии и участников научного проекта.

Во всем мире тысячи женщин и персон с женскими репродуктивными органами ежедневно испытывают боль, связанную с менструацией. Они могут быть немного болезненными, но не должны мешать вам жить своей повседневной жизнью. Эндометриоз - это заболевание, которым страдает примерно каждая десятая женщина. Вместо того, чтобы выводиться через влагалище во время менструации, клетки эндометрия мигрируют в другие органы. Эндометриоз имеет большое количество симптомов, что делает его скорее синдромом, чем болезнью. Неизвестно, как именно он появляется и что вызывает срабатывание. Но одно можно сказать наверняка: это вызывает боль и табу. Психологическая нагрузка также тяжела для пациентов. Что наиболее важно, диагностика эндометриоза требует времени и иногда дорогостоящих, и неточных инвазивных обследований. Это то, что побудило EndoSeek попытаться найти решение этих различных проблем.

Но для завершения исследовательского проекта недостаточно оставаться в лаборатории. На самом деле есть много разных аспектов, которые нужно изучить. Как проект может повлиять на общество? Как пациенты с эндометриозом видят наш проект? Более того, проект может по-разному влиять на общество в зависимости от его применения. Вот почему мы попытались адаптировать наш проект, обменявшись мнениями с заинтересованными сторонами: врачами, писателями и другими.

В ходе наших обсуждений с ними мы адаптировали наш проект благодаря различным практикам, поскольку нам казалось, что по этому поводу не существует глобального консенсуса: действительно, согласно их интервью и обмену мнениями, наш проект действительно развивался в разных направлениях.

Мы сосредоточили наш подход на предоставлении слова тем, кого это касается, потому что нам казалось важным, чтобы эти люди, в основном женщины, но также и ЛГБТ+ персоны, вернули себе слово и дискурс вокруг этой болезни. Эта первая веха в развитии нашей человеческой практики позволила нам узнать о проблемах, связанных с этой болезнью, через призму тех, кто был диагностирован. Фактически, собрав различные свидетельства, мы столкнулись с проблемой, затрагивающей женщин и персон с эндометриозом, более широкой - а именно с минимизацией и упрощением общества боли/симптомов. Некоторые женщины не считают правомерным обращаться к врачу из-за мнения что терпеть боль - это часть нормы в обществе. Кроме того, сексуальность и женское здоровье - темы, которые мало обсуждаются в семье или школе, что способствует определенному невежеству и затруднениям в обсуждении этих вопросов.

Мы получили различные свидетельства, и выяснилось, что время для установления диагноза было очень долгим, в среднем 7 лет с момента появления симптомов. Согласно этим свидетельствам, это связано с различными факторами. Действительно, мы обнаружили, что специалисты ориентировали диагноз больше на другие проблемы со здоровьем.

Среди этих свидетельств всплывает одна мысль:

"если бы я не взяла дело в свои руки, я бы долго ждала".

Это предложение, как правило, указывает на недостаток опыта и, в более общем плане, на то, что система здравоохранения является неполной. «Некоторые врачи общей практики не знают об этой болезни», - говорит женщина. На самом деле это правда, мы видим, что аспиранты-медики изучают эндометриоз только с 2020 года. Существует как медицинское незнание, с одной стороны, связанное с отсутствием опыта, так и с другой стороны, с ошибочными диагнозами, как у женщины, которая разделяет это из-за того, что она была несколько раз неправильно диагностирована под наблюдением нескольких специалистов (гинеколог, радиолог). Кроме того, учитывая сложность постановки четкого и надежного диагноза, несколько свидетельств говорят нам об их энтузиазме в отношении потенциальной молекулярной диагностики, которую может быть развивать.

Эти первые свидетельства стали отправной точкой. Однако последняя не представляла собой достаточно “видимую в обществе” разнообразную исследуемую группу (женское население, пораженное этим заболеванием). Тогда было необходимо узнать мнение более широкой и представительной общественности, а не только женщин, уже страдающих эндометриозом, поскольку болезнь вписывается в более широкий контекст.

Нами был проведен опрос, чтобы узнать больше об эндометриозе в сообществе iGEM. Он позволяет охватить публику, безусловно, посвященную науке, но, прежде всего, международную, смешанную, с разнообразным опытом и кругозором. Всего было собрано 135 ответов. Корреспонденты были из 32 различных стран (Франция, Германия, Индия, Греция, Австрия, Швеция, США, Мексика, Тайвань, Боливия, Испания, Болгария, Эстония, Россия, Канада, Индонезия, Беларусь, Нидерланды, Южная Корея, Бразилия, Великобритания, Гонконг, Австралия, Финляндия, Перу, Швейцария, Турция, Дания, Китай, Литва, Пуэрто-Рико и Азербайджан).

Оказалось, что, хотя немногие iGEM-пользователи (23,1%) не знают о болезни, большая часть (36,6%) слышали о ней, но не знают, что это такое. Поэтому, чтобы проверить, как они понимают что это такое, мы попросили дать краткое объяснение этого заболевания. В общем, люди отвечают, что это заболевание, связанное с маткой, но не раскрывают подробностей. Это может свидетельствовать о том, что респонденты не знают, что именно. Другие ответы представляют собой списки определенных симптомов, но без определения эндометриоза. Кроме того, было дано много ложных ответов: путаница с кровотечением вне менструации, воспалением, кистами, повреждением эндометрия, инфекцией эндометрия, состоянием, из-за которого эндометрий становится толще, чем обычно, порок развития матки, аутоиммунное заболевание и так далее ... Таким образом, эти различные ответы доказывают, что эндометриоз еще недостаточно хорошо известен широкой публике. К счастью, все еще есть несколько правильных ответов. (не из нашей команды)

Команда не только работала над влиянием проекта на общество, опрашивая пациентов, ассоциации и врачей, но и изучала историю эндометриоза в искусстве. Была проведена целая исследовательская работа, чтобы увидеть, как его репрезентуют художники и как общество может видеть эту болезнь через призму искусства и видит ли оно ее вообще. В этом году нам повезло, что у нас есть член команды из Школы Лувра (Ecole du Louvre), который смог поделиться с нами информацией и провести это исследование. Чтобы узнать об эндометриозе не только в научном, но и в человеческом масштабе (поскольку это проект Human Practices), мы провели небольшое исследование того, как эндометриоз был представлен в истории и истории искусств. Мы также провели исследование об эндометриозе в истории от Древнего Египта до 20-го века, которым поделились с другими командами IGEM во время мероприятия Zoom 11 сентября 2021 года, французской встречи. Действительно, одна из целей этой платформы - резко сократить сроки диагностики за счет использования ИИ, согласно анкете, написанной двадцатью французскими экспертами по эндометриозу. Их проходят женщины с тазовой болью, которые задаются вопросом о существовании у них эндометриоза (более 12000 пациентов с января 2021 года). Использование искусственного интеллекта позволит определить вероятность развития эндометриоза, который со временем будет развиваться благодаря используемому «машинному обучению». Это крупный прогресс, который, несомненно, станет результатом научной работы, проводимой по этому вопросу.
Профессор Франсуа Гольфье рекомендует диагностировать, включая одновременно опрос о симптомах, ориентированное гинекологическое обследование и / или МРТ. Молекулярная диагностика может быть полезной, поскольку существуют разные типы эндометриоза. В заключение, все интервью, свидетельства и исследования, проведенные группой Human Practices, позволили нам разработать наш проект EndoSeek так, чтобы он был как можно ближе к заинтересованным людям и привязал его к психосоциальному контексту.
