Endometriosis
Endometriosis is a gynecological pathology whose prevalence in the world population is estimated to be around 10% of women of childbearing age. In addition to women, LGBT + and inersex persons with female reproductive organs can also suffer from endometriosis. Although there are several theories as to the origin of the disease, it is accepted that the disease begins with the proliferation of tissue similar to that of the uterine lining outside of the uterus (Arafah et al., 2021). Distinguish between genital (within the genital organs - uterus, ovaries) and extragenital (outside the reproductive system - the navel, intestines, etc.) endometriosis (Markham SM et al.,1989). Up to 10% of those with the disease may have endometriosis elsewhere in the body, for example on the diaphragm or in the chest cavity (thorax) (APPG on Endometriosis Inquiry Report 2020).

In a healthy body, at each menstrual cycle, in the case of an unfertilized egg, the surface tissue of the endometrium is rejected and excreted from the uterus along with the blood flowing from the vessels of the uterus. In some cases, menstrual blood is thrown into the abdominal cavity through the fallopian tubes, where endometrial cells are grafted and begin to grow. This process gives rise to external endometriosis. With internal endometriosis, endometrial cells "grow" into the muscular layer of the uterus. The most common effects of this pathology on health are pain, increased menstruation, heavy menstrual bleeding, fatigue, pain during or after sex and decreased fertility.

Endometriosis is commonly misdiagnosed and women often report being incorrectly told their symptoms are trivial or normal. (Culley L, et al, 2013) Women suffering from endometriosis see an average of seven physicians before receiving a correct diagnosis, between the onset of symptoms and surgically-obtained biopsies - “gold” standard diagnostic method for detecting endometriosis called a laparoscopy (Rolla, 2019). A 2020 Cochrane systematic review found that for all types of endometriosis, "it is uncertain whether laparoscopic surgery improves overall pain compared to diagnostic laparoscopy" (Bafort C, et al., 2020).

A 20-year study of 12,000 women with endometriosis found that individuals under 40 who are diagnosed with endometriosis are 3 times more likely to have heart problems than their healthy peers. (Wise J, 2016) Endometriosis is a disease with a relatively unpredictable spontaneous course, sometimes with spontaneous regressions.(​​Hickey M, et al., 2014) During pregnancy, there is a regression of endometriosis and women who have been pregnant have half the risk of recurrence than those who have not. The risk of developing ovarian cancer is slightly increased.(Kim HS et al., 2014) As well as the physical impact, the survey demonstrated the impact endometriosis and its symptoms can have on all aspects of life including education, career, relationships and social life, as well as mental health. (APPG on Endometriosis Inquiry Report 2020).

Each affected woman lost on average 10.8 hours of work weekly, mainly owing to reduced effectiveness while working. Loss of work productivity translated into significant costs per woman/week, from US$4 in Nigeria to US$456 in Italy. (Nnoaham KE et al., 2011).

There is a general lack of knowledge about endometriosis among primary care physicians. Half of general health care providers surveyed were unable to name three symptoms of endometriosis.(Quibel A, et al.,2013) We see the gender, race and ethnicity data gaps in research - Endometriosis is less thoroughly studied among Black people, and the research that has been done is outdated. (Shade GH, et al., 2011) Those who do not identify as female may find it distressing to attend appointments in a women’s hospital or dedicated women’s unit, which may mean that they are less likely to seek professional help.
The reluctance for invasive procedures, together with the taboo associated with gynecological diseases, contribute to the long delay in diagnosis, on average 8 years after the first visit to a healthcare professional (Greene et al., 2009).

Endometriose

Endometriosis and COVID pandemy

The COVID-19 pandemic has led to a dramatic shift in the clinical practice of women's health and routine care for endometriosis has been severely disrupted.
Whilst the primary focus was on the care of patients with SARS-CoV-2 and public health measures to prevent transmission/identify those most at risk, individuals with chronic conditions saw their treatments halted, cancelled or changed with very little warning, little information available and extremely limited access to clinicians (in primary or secondary care).

The University of Oxford led a study in collaboration with Endometriosis UK and INSERM, Paris to identify the impact the pandemic has had on people with endometriosis. On question “During the pandemic, what one thing would be most helpful to you, relating to endometriosis? ” (2071 respondents)
- 34.3% of respondents answer “Being able to talk to my gynaecologist”
- 27% - “Knowing when my surgery/fertility treatment will happen”

As restrictions begin to ease and healthcare starts to go back to normal, what one thing do you think should be prioritised with regards to endometriosis? 2098 respondents:
- Arranging all treatments that were cancelled/postponed: 49.8%
- Appointments with gynaecologists: 31.6%

To date, there is no evidence that those with endometriosis are at increased risk of becoming infected with SARS-CoV-2 or developing COVID-19 disease. Similarly, there is no evidence that COVID-19 will hasten the progression/development of endometriosis. (Leonardi M, et al., 2020)

Future Considerations
Firstly, covid there may be an ongoing openness to telehealth. Pandemy could dramatically minimize barriers to care that many women and LGBT+ persons experience, and facilitate the development of endometriosis networks of expertise, which is recommended by the World Endometriosis Society (Johnson NP, et al, 2013). Also it can help as an alternative for patients with unbearable pain that limits their ability to travel to their healthcare provider in some settings. Secondly, there may be increased awareness of self-management strategies that have always existed, yet were under-utilized (e.g., mindfulness, physical exercise, and diet) until COVID-19 resulted in them becoming valuable tools for patients.

Markers for detection of endometriosis
More than ten theories on the origin of endometriosis have been proposed, each of which offers an explanation of its etiology, pathogenesis, and pathological process; but no one theory explains all of the various forms and manifestations of this disease. (Mikhaleva, et al, 2020) A non-invasive diagnostic test could be developed for serum or plasma, urine, endometrial fluid, or menstrual fluid that can be recovered from the posterior vaginal fornix and from the cervix during speculum examination. Broadly, noninvasive diagnostic markers can be made from endometriosis patients compared with menstrual-stage-matched control subjects: glycoproteins, cytokines, immune cell populations, miRNAs, transcriptome, proteome, metabolome, etc. Endometriosis biomarkers are mainly glycoproteins, growth or adhesion factors, hormones, or proteins related to immunology or angiogenesis (Fassbender A, et al., 2011; Othman et al.,2008).

In bibliography we found a lot of possible markers for endometriosis which had different sensitivity and specificity. High levels of VCAN (gene is a member of the aggrecan/versican proteoglycan family) may promote an invasive phenotype of endometrial cells in endometriosis by affecting their proliferation, apoptosis, adhesion, and migration, and also may participate in or be causative of the upregulation of EGFR in endometriosis.(Cattaruzza S, et al.,2002) Some studies demonstrate differences in EGFR expression between stages of endometriosis and also support earlier studies noting involvement of EGF family members in the pathophysiology of endometriosis. (Burney RO, et al., 2007) Interestingly, EGFR is a tumor marker, particularly for epithelial tumors such as colon cancer, lung cancer, prostate cancer, breast cancer, or other solid tumors - Whether it is a marker of the severity of endometriosis remains to be determined.

This initial result reflects that androgen-dependent increases in CA125 and CA19-9 levels are potentially significant diagnostic biomarkers of endometrial pathology [46].

Biomarkers in endometriosis: challenges and opportunities.
As a transcriptional regulator, it is likely that aberrant expression of miRNA translates to dysregulated gene expression in the eutopic endometrium of patients. Therefore, miRNA profiling of eutopic endometrium in endometriosis patients would provide pathophysiologic molecular fingerprints that we can use to further understand disease pathogenesis and as potential noninvasive biomarkers for endometriosis. Potential use of circulating miRNA as a noninvasive biomarker for endometriosis is an ongoing area of research, and its therapeutic and diagnostic implications have been extensively reviewed (https://doi.org/10.1016/j.jmig.2015.02.021, https://doi.org/10.1016/j.ejogrb.2014.10.043, https://www.sciencedirect.com/science/article/abs/pii/S1472648313004501, https://doi.org/10.1093/molehr/gap068 ).

Wang et al. calculated an AUC of 0.994 for the combination of four miRNAs (miR-199a, miR-122, miR-145, and miR-542-3p) [65]. A recent prospective study reported that both miR-199a and miR-122 were upregulated in serum from women with endometriosis, showing high specificity for endometriosis [72], while a different group had reported significant downregulation of miR-199a in the serum of women with endometriosis [66]. The following other circulating miRNAs have been suggested as biomarkers in publications from 2018: miR-17 [126], miR-31 [74], and miR-154-5p [127]. The extracellular matrix proteoglycan versican (VCAN), which regulates cell proliferation and apoptosis, was the most highly expressed gene in severe versus mild disease. Upregulation of microRNA 21 (MIR21) and DICER1 transcripts suggests roles for microRNAs (miRNAs) in the pathogenesis of severe versus mild endometriosis, potentially through regulation of gene silencing and epigenetic mechanisms. These observed differences in transcriptomic signatures and signaling pathways may result in poorly programmed endometrium during the cycle, contributing to lower implantation and pregnancy rates in women with severe versus mild endometriosis.



miRNA production and endometriosis
First discovered in C. elegans as developmental regulators (Saliminejad et al., 2019), microRNAs (miRNAs) are small non-coding RNAs of about 18 to 24 nucleotides involved in the post-transcriptional regulation of the expression of many genes. These small RNAs can be transported from one cell to another via vesicles or associated with proteins, and modulate the translation or stability of mRNAs after complementary base-pairing. They are thought to be involved in the regulation of the expression of about 60% of mammalian genes. In addition, the differential expression of an increasing number of these miRNAs is associated with pathological conditions (Sohel, 2016). Thus miRNAs constitute potential biomarkers for certain pathologies such as prostate cancer (Arrighetti & Beretta, 2021) or endometriosis (Moustafa et al., 2020) (Bjorkman & Taylor, 2019).

The Cas systems for RNAs detection
CRISPR-Cas systems are the adaptive immune system of prokaryotes. They allow bacteria and archaea to keep a trace in their genome of the phages they encountered and to specifically degrade their genetic material in case of infection. These systems, the most famous of which is CRISPR-Cas9, are now increasingly studied for their usefulness in genome editing and as diagnostic tools in biotechnology, agri-food, and health (Nidhi et al., 2021). Most recently, the RNA-cleaving protein Cas13a was used for miRNA detection (Gao et al., 2021).(Fozouni et al., 2021) Cas13 is an outlier in the CRISPR world because it targets RNA, not DNA. Once it is activated by a ssRNA sequence bearing complementarity to its crRNA spacer, it unleashes a nonspecific RNase activity and destroys all nearby RNA regardless of their sequence. This property has been harnessed in vitro for precision diagnostics. These systems can also be used for efficient, multiplexable, and specific RNA knockdown or RNA sequence editing in mammalian cells. This makes Cas13 a potentially significant therapeutic for influencing gene expression without altering genome sequence. ()

Cascade CRIPSR/Cas
To increase test sensitivity, most Cas13-dependent miRNA detection systems rely on target amplification by reverse-transcription recombinase polymerase amplification (Gao et al., 2021). In this project, Team iGEM GO-Paris-Saclay 2021 will test a signaling cascade using Cas13a and Cas14a1 in order to increase detection sensitivity in the absence of target amplification. Inspired by the "Cascade CRISPR/Cas" developed by Sha et al. (2021), our test will rely on the ability of the two Cas proteins to cleave different nucleic acids (Fig. 1): upon association with the target miRNA with the crRNA, the collateral activity of Cas13a will cut the phosphodiester bond between two uracil in the loop of a stable hairpin (ST-HP). The resulting duplex structure can interact with Cas14a1 through strand displacement with the single guide RNA (sgRNA). The resulting activated Cas14a1 nuclease can cleave a ssDNA probe linked to a fluorophore and a quencher, thereby releasing a fluorescence signal.

Les marqueurs pour la détection de l’endometriose
Plus de dix théories sur l'origine de l'endométriose ont été proposées, chacune d'entre elles offrant une explication sur son étiologie, sa pathogénie ou son processus pathologique. Mais aucune théorie n'explique l'ensemble des diverses formes et manifestations de cette maladie. (Mikhaleva, et al, 2020) Un test de diagnostic non invasif pourrait être développé via le sérum ou le plasma, l'urine, le liquide endométrial ou le liquide menstruel qui peut être récupéré du fornix vaginal postérieur et du col de l'utérus pendant un examen au spéculum. En ce qui concerne les marqueurs utilisés par l'équipe iGEM Rochester (https://2020.igem.org/Team:Rochester), nous avons constaté que le CA 125 et d'autres marqueurs de cytokines inflammatoires ne sont pas assez précis et ne sont utilisés qu'à des stades avancés de l'endométriose, ce qui peut être associé à un cancer associé à l'endométriose.(Ahn SH et al.,2017) Cela a également influencé notre choix des microARNs comme marqueur biosynthétique et précis possible.

Wider implications: It is emerging that miRNAs play a central role in the pathogenesis of endometriosis and have the potential of being promising biomarkers. Circulating miRNAs as a non-invasive diagnostic tool may shorten the delay in the diagnosis of the disease, thus alleviating the suffering of women and reducing the burden on health care systems. However, despite numerous studies on circulating miRNAs in endometriosis, no single miRNA or any panel of them seems to meet the criteria of a diagnostic biomarker. The disagreement between the various studies upholds the demand of larger, well-controlled systematic validation studies with uniformity in the research approaches and involving diverse populations.

Wang et al. calculated an AUC of 0.994 for the combination of four miRNAs (miR-199a, miR-122, miR-145, and miR-542-3p) [65]. A recent prospective study reported that both miR-199a and miR-122 were upregulated in serum from women with endometriosis, showing high specificity for endometriosis [72], while a different group had reported significant downregulation of miR-199a in the serum of women with endometriosis [66]. The following other circulating miRNAs have been suggested as biomarkers in publications from 2018: miR-17 [126], miR-31 [74], and miR-154-5p [127]. Upregulation of microRNA 21 (MIR21) and DICER1 transcripts suggests roles for microRNAs (miRNAs) in the pathogenesis of severe versus mild endometriosis, potentially through regulation of gene silencing and epigenetic mechanisms. These observed differences in transcriptomic signatures and signaling pathways may result in poorly programmed endometrium during the cycle, contributing to lower implantation and pregnancy rates in women with severe versus mild endometriosis.

However, despite numerous studies on circulating miRNAs in endometriosis, no single miRNA or any panel of them seems to meet the criteria of a diagnostic biomarker. The disagreement between the various studies upholds the demand of larger, well-controlled systematic validation studies with uniformity in the research approaches and involving diverse populations.

Production de miRNAs et endometriose
écouverts pour la première fois chez C.elegans comme régulateurs du développement (Saliminejad et al., 2019), les microARNs (miRNAs) sont de petits ARNs non codants d'environ 18 à 24 nucléotides impliqués dans la régulation post-transcriptionnelle de l'expression de nombreux gènes. Ces petits ARNs peuvent être transportés d'une cellule à l'autre via des vésicules ou associés à des protéines, et modulent la traduction ou la stabilité des ARNms après appariement de bases complémentaires. On pense qu'ils sont impliqués dans la régulation de l'expression d'environ 60 % des gènes de mammifères. De plus, l'expression différentielle d'un nombre croissant de ces miRNAs est associée à des conditions pathologiques (Sohel, 2016). Ainsi, les miRNAs constituent des biomarqueurs potentiels pour certaines pathologies telles que le cancer de la prostate (Arrighetti & Beretta, 2021) ou l'endométriose (Moustafa et al., 2020) (Bjorkman & Taylor, 2019).

Les systèmes Cas pour la détection des ARNs
Les systèmes CRISPR-Cas constituent le système immunitaire adaptatif des procaryotes. Ils permettent aux bactéries et aux archées de garder une trace dans leur génome des phages qu'elles ont rencontrés et de dégrader spécifiquement leur matériel génétique en cas d'infection. Ces systèmes, dont le plus célèbre est CRISPR-Cas9, sont aujourd'hui de plus en plus étudiés pour leur utilité dans l'édition du génome et comme outils de diagnostic en biotechnologie, agroalimentaire et santé (Nidhi et al., 2021). Plus récemment, la protéine clivant l'ARN Cas13a a été utilisée pour la détection des miRNAs (Gao et al., 2021).(Fozouni et al., 2021) Cas13a est une exception dans le monde CRISPR car elle cible l'ARN et non l'ADN. Une fois qu'elle est activée par une séquence d'ARN complémentaire de son espaceur d'ARNc, elle libère une activité RNase non spécifique et détruit tous les ARN proches, quelle que soit leur séquence. Cette propriété a été exploitée in vitro pour des diagnostics de précision. Ces systèmes peuvent également être utilisés pour la destruction efficace, multiplexable et spécifique d'ARNs ou l'édition de séquences d'ARN dans les cellules de mammifères. Cela fait de Cas13a un outil intéressant pour influencer l'expression des gènes sans modifier la séquence du génome. (Xu, C.,2021).

La Cascade CRISPR/Cas
Afin d'augmenter la sensibilité du test, la plupart des systèmes de détection de miRNAs dépendant de Cas13a reposent sur l'amplification de la cible par une RT-RCA (reverse-transcription recombinase polymerase amplification)(Gao et al., 2021). Pour ce projet, l'équipe iGEM GO-Paris-Saclay 2021 testera une cascade de signalisation utilisant Cas13a et Cas14a1 afin d'augmenter la sensibilité de détection en l'absence d'amplification de la cible. Inspiré de la " Cascade CRISPR/Cas " développée par Sha et al. (2021), notre test reposera sur la capacité des deux protéines Cas à cliver différents acides nucléiques (Fig. 1) : lors de l'association du miRNA cible avec le crRNA, l'activité collatérale de Cas13a coupera la liaison phosphodiester entre deux uraciles dans la boucle d'une structure en épingle à cheveux stable (ST-HP). La structure duplex résultante pourra interagir avec Cas14a1 par déplacement de brin avec l'ARN guide unique (sgRNA). La nucléase Cas14a1 activée qui en résulte pourra cliver une sonde d'ADN simple brin liée à un fluorophore et à un quencher, libérant ainsi un signal fluorescent.
